Step 1 – Filling out the request header
Drawing up a document begins with filling out the “header”, which is placed in the upper right corner of the A4 sheet. It is advisable to indicate your exact date of birth, since to carry out an investigation into the complaint, a specialist from the Ministry of Health requests medical documentation from the medical institution. Identification of the applicant among persons with similar full names is carried out by date, month and year of birth.

The main part of the appeal sets out the essence of the conflict. The citizen indicates the following information:
- circumstances of receiving medical care;
- position and full name of the medical worker;
- a list of violations and a description of illegal actions;
- references to the rules of law that are violated.
The text should be concise and succinct. If the application is written by hand, you need to make sure that the handwriting is legible and easy to read.

The complaint ends with specific demands for an inspection of the medical worker. If a fee is illegally charged for free medical care, the patient has the right to ask for assistance in returning the money.
At the end, a list of applications is compiled, which includes supporting documents. Then the applicant’s signature and the date of filing the application are placed.

Drawing up a complaint
Any complaint, including a complaint to the Ministry of Health, contains a standard set of structural elements, namely:
- name of the body to which the complaint is sent. In our case, this is the Ministry of Health of the Russian Federation;
- Full name, address, contact details of the applicant;
- a descriptive part that gives an idea of how and for what reason a controversial legal relationship arose: I was in the hospital, called an ambulance, did not find the right medicine, etc.;
- a reasoning part explaining why the applicant believes that his rights were violated;
- the operative part, which includes a request. For example, “I ask you to oblige the head physician of the clinic in the city of N to give me a referral for a free diagnosis” or “I ask you to oblige the head physician of the hospital in the city of N to cancel the order imposing a disciplinary sanction on me”;
- a list of attachments, that is, documentary evidence of a violation of rights;
- date of filing the complaint and signature of the applicant.

Methods for submitting an application to the MZRK

So, the ways to file a claim are as follows:
- Electronic. To do this, you need to go to the website page in Menu-General information-work with the population, where you select the tab to submit an application through the e-government portal. Especially for the convenience of smartphone users, consumers are invited to download the egov mobile application, which will simplify the appeal procedure.
- At a personal reception. The Ministry must necessarily provide for a method of personal communication between the population and officials. Particularly important officials receive appointments according to the established schedule; these employees are the minister, his deputies, and the executive secretary. As a rule, personal appointments are held at least once a month. The schedule of hours and days for the visit must be made known to people by the responsible staff of the apparatus. The schedules are compiled quarterly, they can be found on the ministry’s website, in the same column working with the population, and they are also posted in the government building. It is necessary to prepare well for a personal meeting; you should prepare questions that concern the visitor in advance. If the problem concerns an incident or a large amount of documentation is attached to the application, the applicant has the right to write a statement and then he will be given a written response. This format is also used when the letter requires more detailed consideration, or if circumstances need to be verified.
- The Ministry of Health website contains numbers where a person can call and talk about their problem; each locality has its own helpline.
- Send a letter by mail. The applicant must state his demands, concerns and complaints on paper and send by mail to the address of the ministry or structural unit (department) in whose department the problem is located. To ensure that the letter is not lost, you should send it in the recommended way with notification. Then, after receiving the letter by the authority, the sender will receive the appropriate paper confirming the fact of transmission.
There is also an interesting question-answer function in electronic form.
Appeals in the section are grouped according to thematic principle:
- quality of medical services;
- organization of medical care;
- provision of medicines free of charge.
After the subject in the next column, the user can read the full text of the complaint and click on the “response” link to obtain the contents of the authority’s response letter.
